This reader has been leaving interesting comments on my site but typical from those who hate us conservatives more than the Radical Jihadists. For the Record I do not hate Liberals nor Jihadists but I will defend myself against Jihadists if need be and pray for both the misguided but well intention Liberal and Jihadist...

Kosovo and Iraq are two completely different situations you boob. In Kosovo there was a complete breakdown in the region and war and genocide were spreading rapidly killing innocent people. A clear humanitarian need to intervene existed then and apparently now too.
But, in Iraq a dictatorial maniac ruled and ruled basically in a peaceful society. When Hussein was committing genocide against the Kurds in the 80s, a time when we should have stopped him, the USA under Reagan and Bush Sr. looked the other way since he was engaged in a war with Iran. There are those famous pictures of Rumsfeld shaking hands with Hussein. We gave him weapons and money. Buddy buddies they were then even though it was well documented what Hussein was doing was morally reprehensible. Then after his invasion of Kuwait, he gets rid of his weapons of mass destruction and was trying to loosen up the international restrictions on oil sales. A dangerous nut perhaps, but no longer armed with deadly weapons and staying within his borders... so of course GW has to attack.. Now the Iraq region is a real mess... a mess that the USA created, not the international community... so why should the international community get involved when the US would not listen when they said not to invade. Why would the international community help out when the US ignored them.

We currently have King George back from the days of the revolution and we need to reject him as vehemently as our forefathers did. This war is an atrocity perpetuated by a man that lied to the American public on the reasons to justify an unprovoked attack on another country. Nowhere is the lack of moral authority more clear than this brazen take over of the American democratic process by a group of people who hijacked and rigged elections and then started an unprovoked war. I may disagree with my fellow Americans from the opposite side of the political spectrum from me but I do not want any harm to befall them nor do I want to resort to ad hominem attacks(typical of them although this is minor one by their standars). It is a crying shame that the Left uses personal attacks against us, their conservative countryman.  It seems as if the Left would rather have "OUR"  President taken our rather than Osama and those of his ilk.  How sad for we are all Americans and we have joined forces in the past when faced with Global Enemies during our short history.  The Jihadists are a new kind of enemy and a particularly pernicious, cunning and ruthless one who want to enslave us as Dhimmi at best or decapitate us regardless of whether we are Conservative or Liberal. We Liberals and Conservatives are not ENEMIES but sometimes I wonder if the Left thinks that we are the Enemy. The problem is not our Politics but our Nationality and Religion!

The concept of primary

• (the cause including Trauma, Tumor, Stroke-Ischemic Hemorrhagic) 

vs secondary injury is an essential component of cerebral resuscitation strategies. Neurons do not regenerate--once damaged and killed, they are gone. When they are damaged or die, they release excitatory amino acids, free radicals, leukotrienes, etc (LONG LIST), and these substances damage surrounding neurons and glia, and alter the integrity of the blood brain barrier, which can lead to vasogenic edema. In addition, local or generalized swelling/increased ICP can interfere with cerebral blood flow and cause ischemic damage.

• There is virtually nothing that can be done to reverse the primary injury. Our focus during resuscitation and ongoing management is to prevent, as much as possible, secondary injury.

HOW SERIOUS ARE STROKES?

A stroke is always serious. Unlike cells in other parts of the body, those in the brain cannot be repaired or regenerated. It is the third leading cause of death in the US. About 160,000 Americans die of stroke each year, although mortality rates are declining. Studies have indicated that one-third of all strokes are fatal within the first 30 days.

Ischemic Versus Hemorrhagic Stroke. People who suffer ischemic strokes have a much better chance for survival than those who experience hemorrhagic strokes. And among the ischemic stroke categories, the greatest dangers are posed by embolic strokes, following by large-artery (thrombotic) and lacunar strokes.

Hemorrhagic stroke not only destroys brain cells, but it poses other complications as well, including increased pressure on the brain or spasms in the blood vessels, both of which can be very dangerous. (In one small study, however, survivors of hemorrhagic stroke had a greater chance for recovering function than those who suffered ischemic stroke.)

• Indeed this is a rule of thumb I learned and remember from training and good news for not only Senator Tim Johnson but obviously and Hemorrhagic Stroke victim.  If one survives the initial neurologic insult they have a better chance of recovering function.  However, this is dependent on a number of factors including but limited to:
• Intracranial Pressure spikes
• hypo-perfusion of the Brain
• Infections
• Cerebral Edema
• Location in the Brain of the Hemorrhage
• Re-bleeds
• Free Radical Formation
• From University of Maryland Medical Center
• Cell Death. Other processes in the brain compound the damage. Researchers are working to discover the cycle of events involved in brain cell death after oxygen is lost.
  One hypothesis is as follows:
  • When blood is cut off in the brain, proteins known as excitatory amino acids (such as glutamate or glycine) are released.
  • They open channels in the membranes that cover neurons (brain cells) and allow large amounts of calcium to flow in.
  • Calcium reacts within the neurons to release harmful substances that damage cells.
  Natural particles released by the body's chemical processes called oxygen-free radicals may also play a role in the cell damage caused by a stroke. (Some oxygen-free radicals under study are called lipid peroxides.)

Factors Affecting Recurrence. The risk for recurring stroke within five years is about 22%. The risk for recurrence for any stroke is highest within the first six months, so preventive measures should be instituted as soon as possible. Some specific risk factors for early recurrence are the following:

• Being older.
• Hemorrhagic or embolic stroke.
• Diabetes.
• Alcoholism.
• Valvular heart disease.
• Atrial fibrillation.

Long-Term Outlook for Survivors. Over half of stroke victims survive beyond five years. In general, however, a third of those who survive either an ischemic or hemorrhage stroke suffer some neurologic problems, and nearly all have at least some injury in the brain. On the encouraging side, about 90% of stroke survivors experience varying degrees of improvement after rehabilitation. Furthermore, one 1998 study reported that people who survived for many years after a stroke had a chance for independent living that was about the same as for their peers who had not suffered strokes. The stroke patients even appeared to be less depressed.

 

Loosing Brain Cells is obviously a bad bad thing.  The estimates of the number of brain cells humans are blessed is 100 billion Neurons. Obviously attrition does occur with age, as these brain cells die they are not replaced. With out any neurologic accidents we permanently lose about 100,000 daily, or close to 2 billion in half a century. Brain Injuries are very very bad and noxious conditions to survive and recover from to just carry out ADL's(Activites of Daily Living) working as an Executive at the Nations Capital is an entirely other matter.  Traditions are nice for Senatorial Princes and Princesses but we live in DANGEROUS TIMES and the Federal Government needs all the Brain Cells it can muster to steer the mines we face whether they be conservative or liberal minds is beside the point!  But what do I know as I have been rehabilitating injured minds for over a quarter century?...e=?~

The post you wrote about my opinions regarding the AVM came up so I responded to it. I do not believe that having the Vice President break any tying votes in the Senate will make much of a difference in the Political direction of our country. For 14 years the GOP has been in control of both Houses. Did the GOP perform? Well if you ask many Conservative Americans we have been very disappointed in the Political Firepower of the GOP Majority. I give the GOP a D+ in all areas except for the Supreme Court Appointments where I would give them a B+. Yes I am a Conservative American but I have 21 years of experience with Brain Injury and what I am hearing does not sound good. He is on a ventilator at night and still in a Critical Care sitting or (ICU) 23 days after his surgery and that is not good. The Neurosurgeon indicated that the Angiogram indicates he does not need "Surgery AT THIS TIME". His doctor did not say he will not need surgery in the FUTURE!

"It probably is not over," Dr. William Bank(who treats AVMs and other neurovascular disorders at Washington Hospital Center) added. "For a complete removal of an AVM, you need to be doing your surgery under ideal circumstances," not when the defect is actively bleeding.

South Dakota have your Governor appoint a Democrat and that well be better than this situation. A Democrat who has the ability to think will be better for the country than one who has had brain damage to the extent of our Senator from South Dakota. Of course my opinions must be tempered by the fact that I am not his Attending Physician but I do have the ability to make inferences based on what has been released to the press. The good Senator was recently moved from the ICU, on the ventilator part time and minimally responding to the family and Physicians more than 3 weeks out from the injury. That is not a good sign!

18-30% of people with an AVM bleed have a re-bleed within the first year, sorry if that offends you. Have your Republican governor, Mike Rounds, appoint a Democratic replacement as this will more likely be better than hanging on to Tim Johnson, sorry it that offends you. The GOP members of the Senate for the most part are kinda weak and did not conduct themselves in a robust manner while in the majority so a 50-50 split will not change much in my opinion, sorry if that offends you. I marvel at the robust strength of Schumer, Durbin, Reid, Kennedy, Boxer and Kohl when they were in the minority and fought hard for every inch and only wish the GOP Senators were fighters like them.

I could be wrong and I hope so but the "Preponderance of the Evidence" as floated by the Physicians and Media, my professional knowledge and experience cause me to believe that the prognosis for full recovery is poor. Additionally a medium level of proof or standard known as clear and convincing evidence is satisfied by what I have read about the Senator's progress to date. Good for the Senator and his family as it sounds like he is out of the woods medically but I still do not think he should serve Americans after he has suffered a severe Neurologic Injury aka Brain Damage. In many states his Physician would be required to alert the Department of Motor Vehicles of a patient with a Brain Injury. However in this situation he is appointed to the Chairmanship of Appropriations Committee's subcommittee on military construction and veterans affairs. This is inappropriate folks but the Democrats want power so BAD it is okay? I wish Senator Johnson and his family God's speed! America still needs a capable and non-Brain Injured Senator to work our Capital for not only South Dakotans and all Americans as well....

The Left and its Vietnam Strategy for the US Military in Iraq

The goal of the Left is to castrate the powerful US Military! If the Jurrassic Main Stream Media and its cohorts(Elites, Democrats and Academics)are successful at portraying Iraq as a failure like they did with Vietnam they will achieve this goal. I believe the Left's rational is that if they stigmatize the concept of war in this negative manner any future US President will forever be reluctant and will forever be gun shy to use our Military. The concept of the Commander in Chief's role will be so weakened that we will in effect become a Parliamentary system with an Executive Branch that is Psychologically damaged and scarred over the concept of War. The Military will be relegated to act only on behave of the United Nations. Remember John Kerry said he wanted this years ago to Harvard University The Crimson:

Barnett has great ideas and a great presentation but his support of John Kerry for president and complaints about President George Bush making to many enemies does not chive with good and innovative ideas. John Kerry would never support his Military Leviathan force ideas(which reminds me of Secretary of Defense Donald Rumsfeld ideas) and the World will not cooperate with the System Administration Peace keeping Force and the Al Qaeda types are coming out of the wood work because they have always been there and Europe(which is dying our "Eurabia") will not fight back and is "Mad at Us"!

Barnett has good ideas but I think he his in love with his fancy "set point and connectivity" jargon. His tone is too PC. The Islamic World and former USSR are filled with uncivilized and cunning, heartless people. Again I like his words but we need a strong Navy, Air force, Air craft carriers and Star Wars as these people will take us out when we let our guard down. Economics is only part of the answer the other part is Peace Thru Strength ala President Ronald Reagan!

I think Barnett has great ideas but they are only half baked because he is in his heart a Poli Sci guy and defers to the UN types! If you follow the stream his consciousness you end up steering away from the liberal MSM, UN Elites and Democrats. The Dems will never let his ideas be but into practice. The Islamic Jihad threat does not eminate from poverty but fanatacism. Osama bin Laden was a multimillionaire, educated, globalized and well connected man before he got fanatical.

Barnett has fallen in love with his work and votes for Democrats. I say lets take TPM's ideas and pack then in Semper Fi wrapping paper and stay strong. I would not want to be in a gang fight with TPM!

Nonetheless this interview/book club between Hugh Hewitt and smart TMP Barnum(oops-pun intended beacuse his votes for Democrats is Circus-like) but off the mark Barnett is very interesting and educational!
